Background:

During 2013-18, under the framework of National HIV Response, UNICEF Bangladesh supported AIDS/STD Programme to establish comprehensive prevention of mother-to-child transmission (PMTCT) interventions along with MNCH services in five tertiary medical college hospitals in Dhaka, Chittagong, Sylhet, Khulna and Cox s Bazar. PMTCT interventions were the pioneer HIV interventions in public health system of Bangladesh. The intervention was part of a national PMTCT strategy which emphasized a dual approach based on country HIV epidemiology. The PMTCT interventions allowed to gain significant experiences and expertise and open the scope for introduction/consolidation of broader HIV services in tertiary hospitals.

From 2017, government supported introduction of comprehensive care and treatment (CST) services for people living with HIV (PLHIV). That was the transition effort by the GoB to effectively use public health system to deliver treatment, care and support services for PLHIV. So, GoB requested UNICEF s support for preparing the hospital and creating an enabling environment for complete transition of treatment, care and support services. It was decided that experiences of PMTCT services could be used to further strengthening treatment, care and support services in hospitals.

To take forward the HIV services from tertiary hospitals, Bangladesh, AIDS/STD Programme (ASP) will require support in the following areas; assessment of preparedness of the hospitals for PMTCT/ART, building capacity of service providers on ART/PMTCT, setting up HIV Testing sites, training on HTC, Behavior Change Communication, stigma and discrimination, use of communication materials, psychosocial counselling, developing reporting system and M&E, developing mechanism of interdepartmental coordination, providing onsite TA to hospitals. These technical supports will be crucial for developing sustainable HIV services from tertiary hospitals in Bangladesh.

Purpose of Activity/Assignment:

To provide technical support to AIDS/STD Programme (ASP), five tertiary hospitals with PMTCT interventions and National ART/ PMTC Task Force for initiating, consolidating and implementation of comprehensive HIV/PMTCT services in 5 tertiary hospitals. The specific objectives include:

Capacity building of clinicians, service providers on ART/PMTCT management, HIV testing, Communication, stigma and discrimination through organizing, facilitating and conducting trainingsProvide onsite technical assistance as and where necessaryDevelop M&E, data collection and reporting system

Deliverables/Outputs

Facilitating updating of National PMTCT guideline; Organize basic orientation on HIV, PMTCT and stigma and discrimination for all staff of SSSADH; Organize refresher training on Counselling and Testing in KMCH; Facilitate training on ART/OIs management for service providers.Organize training on HIV testing services for nurses/midwives in SOMCH including SSSADH; Organize basic orientation on HIV, PMTCT and stigma and discrimination for all staff of MDH.Organize training on HIV testing services for nurses/midwives in MDH; Organize training on universal precautions for health care providers in KMCH; Organize training on universal precautions for health care providers of SSSADH; Organize training on universal precautions for health care providers of MDH.Facilitate training on PMTCT and syphilis management for service providers in KMCH; Organize training on post-exposure prophylaxis for key staff of SSSADH; Facilitate training on PMTCT and syphilis management for clinical service providers in SOMCH including SSSADH.Facilitate training on PMTCT and syphilis management for clinical service providers in MDH.Organize training on post-exposure prophylaxis for key staff of MDH; Conduct training on breast feeding for key staff in SSSADH.Conduct training on breast feeding for key staff in MDH.Develop consolidate PMTCT report development.Organize PMTCT review meeting in Dhaka.Conduct training assessment for 2020.Develop consultancy completion Report.20 field visits - Travel / DSA (actual as per UNICEF policy).

Knowledge/Expertise/Skills required:

Education:An Advanced University degree in public health, or related field with technical knowledge in clinical management of HIV/AIDS in women and children; Master s in Psychology; MBBS.

Experiences: At least Eight (8) years expertise with five (5) years on clinical counselling, HIV Counselling and Testing and exposure to ART/PMTCT management.

Skills and attributes: Effective communication skills, experience in analysis, advocacy skills, report writing skills.

Language proficiency:Excellent spoken and written English.

Professional Fee range:BDT 200,000/= per month (22 working days)

TA/DSA :Travel and DSA will be provided as per UNICEF Bangladesh policy.
